No. The NO-COST Accidental Death coverage and, if elected, the Expanded Death coverage are extended to you on a guaranteed basis. There is no long form with complicated health questions to worry about. No medical exam is required.

You and your spouse are GUARANTEED ACCEPTANCE in the NO-COST and Expanded Accidental Death coverages … regardless of your health history, current occupation, or hobbies.

We'll send you a Certificate of Insurance. Look it over for a full 30 days. If you're not completely satisfied, just let us know. We'll cancel your coverage and give you a complete 100% refund of any premium.

Coverage for all insureds (whether member or spouse) will reduce by 50% when the Member reaches age 75 regardless of age at enrollment.

Benefits for accidental loss of life are payable to your beneficiary. If no beneficiary has been named, benefits will be paid to: your spouse; your children; your parents; your siblings; or your estate (in that order).

Benefits will not be paid for intentionally self-inflicted injury, suicide, war, military combat activities, sickness/disease, certain aviation activities, intoxication-related incidents, or professional racing activities.
